Inconsistencies with request_review #56561

create-pull-request has a default of request_review but that is non-idiomatic, the name should be request-review to be consistent

push-to-pull-request-branch doesn't have a request_review option - it shoul have this and this should be the default.

CHANGELOG.md should be excluded by default as all PR creation realistically has to update the CHANGELOG.md most of the time.

Update docs as well
